Our office participates in various commercial insurance plans. Some insurance companies require precertification for certain procedures. Please contact your insurance company prior to your visit regarding this matter. Please bring your current insurance information when visiting our office. Any expenses incurred with Eyvazzadeh & Reilly Colon and Rectal Center are the patient’s responsibility. However, we do extend the courtesy of submitting our patients’ claims.
We try to provide access to our practice to everyone. If you find your insurance company is not represented on our list, please contact our Practice Administrator to see if we might become participants in your program. Our aim is to please you.
To reduce administrative cost, we ask that all non-covered fees including co-payments and office visits be paid by cash or check at the time of the appointment
Eyvazzadeh & Reilly Colon and Rectal Center is a participating provider with Medicare. Our office receives 80% of the approved amount from Medicare. Please inform us if you have secondary insurance to cover the additional 20%. Medicare requires us to submit all charges. The patient is responsible for the yearly deductible payment.
